Instruction for Membership

NEW MEMBERSHIP
  
1. Please download the MEMBERSHIP APPLICATION FORM
2. Complete the following:
    * Attached a recent passport size photograph.
    * Membership details.
    * Membership period.
    * Membership fees.
3. Effect payment payable to Pacesetters Athletic Club, RHB account # 1-14035-0054088-7 via
    * Cash
    * Cheque
    * Online banking
4. Scan and email or post the duly completed membership application form together with deposit slip or online
    transfer details to the Club's Admin. Manager.

NOTE
a] If you are not sure how much you should pay, please email (preferred method) or call our office to verify the correct amount before payment is made.
b] Your membership will be accepted once the exact amount and full details are received. 
c] Please allow 4 weeks for the membership card to be issued to the mailing address.

MEMBERSHIP RENEWAL
  
1. Please download MEMBERSHIP APPLICATION FORM
2. Complete the following
    * Tick renewal section.
    * Membership details if differs from previously provided.
    * Amount payable.
    * Duly sign.
3. Effect payment payable to Pacesetters Athletic Club, RHB account # 1-14035-0054088-7 via
    * Cash
    * Cheque
    * Online banking
4. Fax or scan and email the duly completed membership renewal form together with the deposit slip/on-line  
    transfer details [name/date of payment & amount] to

NOTE
[a] PHOTOGRAPH is not required for renewal of membership.
[b] Expired membership will have their Footloose delivery and benefits discontinued.
[c] Please renew to avoid inconvenience and disruption of service.
[d] A renewal sticker to attach on the card will be sent to you.

INCENTIVE FOR THREE [3] YEARS MEMBERSHIP SUBSCRIPTION
  
* NEW or EXISTING members who subscribe for a three [3]-years membership period will be presented with a
  Club merchandise. This incentive is available while stock last. 
* Members are REQUIRED to present the original official receipt as proof of the 3-year membership subscription
  to redeem the merchandise.
* The merchandise can be collected from PACM office during office hours or events organised by PACM
* It is redeemable within TWO [2] months from the date of official receipt issued.

CARD REPLACEMENT
  
* A member who has lost their card or requires a new card during renewal is required to pay a fee of RM8.00. 
* The procedure is similar to New Membership Application.

MEMBERSHIP UPDATE
  
* Members are encouraged to maintain an up-to-date data with the Club to facilitate the dissemination of information.
* If there are any changes to your postal address, contact number or email address, kindly email the Club's
  Admin Manager and to include
  * Member's name
  * Membership number
  * Details of the changes
* This is vital so that you can continue to receive the Club's quarterly Footloose magazine, eBulletin or notices
  through post or email.
